You can assign question forms to service request types so that when you create service requests of this type in the mobile app you can answer questions from assigned question forms.
In the Service Request Type dialog box, click the Question Forms tab, move the appropriate question forms from the Available Question Forms box to the Assigned Question Forms box, and then click OK.
When you create or edit service requests of this type you will see the questions from assigned question forms on the Questions screen.
